STEP 2 Submit an application form in person


Category A	Establishment of identity (e.g. Australian birth certificate)
Category B	Link between identity and person (e.g. Australian passport)
Category C	Evidence of identity in community (e.g. Tax File Number Letter)
Category D	Proof of residence (e.g. Letter showing current residential address)

You will need to take an eye test when you apply to upgrade your licence.

If you need to wear glasses or contacts to pass the eyesight test, your licence will have the condition that you must wear them when driving.
