Replace your driver's licence
Find out how to replace a lost, damaged or stolen WA driver's licence.
You can apply for a replacement licence if your driver’s licence is lost, damaged or stolen.
You can also replace your licence for other reasons, such as adding a new vehicle class.
You cannot apply for a replacement driver's licence if:
- you have been prohibited from driving a motor vehicle or riding a motorcycle in WA or elsewhere since last obtaining your licence
- your licence has been disqualified, cancelled, suspended, expired or there are charges pending against you
- you are subject to an appeal for driving or riding offences.
Replace a lost or damaged driver's licence
If your driver’s licence is lost or damaged, you can apply for a replacement:
- online using DoTDirect
- in person at a Driver Vehicle Services (DVS) centre, DoT regional office or agent.
- by post.
You will need to visit a DVS centre, DoT regional office or agent, if you:
- need to change your name on your driver’s licence
- need to update your photo or your licence is over 10 years old
- have changed your address in the last 7 days
- have ordered a replacement licence online in the last 12 months.
Concession holders
If you are eligible for a 100% concession on your licence renewal fee, you are eligible for a free replacement driver’s licence.
Apply online
To order a replacement licence card online, you will need to:
- Log in to your DotDirect account.
- Select the option under the 'Driver's Licence or Tools ' tab and follow the prompts.
- Pay the replacement fee.

Apply by post
To order a replacement licence by post, you will need to provide:
- a completed Application for Replacement/Certified Copy of Driver's Licence, Learner's Permit or Learner Log Book (DL26) form
- certified copies of primary and secondary proof of identity
- cheque, money order or credit card details for replacement fee.

Replace a stolen licence
If your WA driver’s licence has been stolen, you need to report it to the Western Australian Police Force.
Once you have a WA police report number, you will be eligible for a free certified copy of your licence.
To replace your stolen licence, you will need to visit a DVS centre, DoT regional office or agent or apply by post.
We can only accept a WA Police report.
WA Driver’s licence documents stolen interstate or overseas are not eligible for a free certified copy.
If your driver’s licence is stolen interstate, or overseas, we cannot accept an interstate, or overseas police report to provide a free replacement driver’s licence.
Apply in person
You will need to visit a DVS centre, DoT regional office or agent and:
- Submit a completed Application for Replacement/Certified Copy of Driver's Licence, Learner's Permit or Learner Log Book (DL26) form.
- Provide original copies of primary and secondary proof of identity.
- Provide a WA police report number.

Replace a Driver’s licence (including extraordinary licence) learner’s permit, Log Book, and WA Photo Card due to a declared natural disaster
If your Driver’s licence (including an extraordinary licence) learner’s permit, Log Book, and WA Photo Card have been damaged or destroyed as a direct result of a hazard that has been declared an emergency situation or state of emergency, you may be eligible for a free replacement of the eligible documents.
A hazard means, but is not limited to, a cyclone, earthquake, flood, storm, tsunami or other natural event as defined under the Emergency Management Act 2005.
An emergency situation means a declaration made under section 50 of the Emergency Management Act 2005.
A state of emergency means a declaration made under section 56 of the Emergency Management Act 2005.
There is no time limit on the fee exemptions; however, these requests must be related to a hazard declared as an emergency situation or state of emergency.
To obtain a free replacement of your Driver’s licence, learner’s permit and/or WA Photo Card you must apply in person.
You will need to visit a DVS centre, DoT regional office or agent.
- Return the plasticised document to DoT for destruction if the card is damaged.
- Complete the Declared Emergency Situation Damaged Documents/Plates Declaration (E47A) form which includes a statutory declaration stating that the document was damaged or destroyed because of the natural disaster.
- Provide identification as outlined on the Declared Emergency Situation Damaged Documents/Plates Declaration (E47A) form.
- Complete the Verification of Identity for Driver's Licence or Photo Card Application (E46) form if all acceptable forms of identification have been destroyed as a direct result of the natural disaster.
- Have your photo and/or signature captured (if required).
If you have had your printed Log Book damaged or destroyed and you are seeking recognition of your supervised driving hours already taken you must have your supervising driver complete a Statutory Declaration.
