Change my name
If you’ve changed your name, you need to let us know. Find out how to update your personal details with the Department of Transport (DoT).
If you’ve changed your name after marriage, the Registry of Births, Deaths and Marriages has information on how to register your name change.
What you need to do
You can update your name in person at Driver and Vehicle Services centre, DoT regional office or agent . You'll need to bring:
- a completed Change of Personal Details (Form C4)
- primary and secondary proof of identity of which:
- 1 is primary and
- 1 is secondary
- originals of any documentation relating to your name change e.g. marriage, change of name or birth certificate issued by Registry of Births, Deaths and Marriages or Freedom of Information (FOI) decision letter.

Please note:
- your identity and change of name documents should show a clear link between your names
- at least 1 of the documents should contain your signature
- commemorative marriage certificates will not be accepted.
Replacement card or document fees
You don’t need to pay a fee to update your name, address or gender.
You’ll need to pay a fee if you request a replacement driver's licence card, WA photo card, vehicle licence document, or learner’s permit card.
