The WA Photo Card is a recognised form of identity for Western Australians aged 16 years or over. It’s a voluntary card to assist people who live in WA, who don’t hold a driver's licence or passport, in proving their identity.
Cardholders have the option of including their address on the card and can choose to have 2 cards – 1 showing their address and 1 without their address.
WA Photo Cards are valid for 5 years.
Fraudulent applications will be referred to the Western Australia Police Force.
How to apply
To apply for a WA Photo Card, you’ll need to visit a Driver and Vehicle Services (DVS) centre, regional DoT office or agent in person.
You’ll need to bring:
- a completed Photo Card Application (Form PC1)
- proof of identity:
- First time applicants: You’ll need bring with you 5 points of identity. See Identity requirements for driver's licence, learner's permit or WA photo card.
- Holders of a Driver’s licence or Learner’s permit: You’ll need bring 1 primary and 1 secondary proof of identity. See Identity requirements for existing customers and other transactions.
You’ll need to pay an application fee, and have your photo taken.
If you do not receive your WA Photo Card within 21 days of application, please contact us.
Submit WA Photo Card Application by post
You can submit your WA Photo Card Application form by post if you’re unable to have your photo taken at a Driver and Vehicle Services (DVS) centre, regional DoT office or agent because you:
- have a medical condition preventing you from going to one of our locations
- are living in a remote area within WA
- are travelling interstate or overseas.
Please download and complete the Photo Kit (E13) form, which includes detailed instructions and the mailing address.
WA Photo Card security
WA Photo Cards have the same level of security as a driver's licence and display the following information:
- name
- date of birth
- signature
- photo
- address (optional).
Just like a driver's licence, your WA Photo Card holds important personal information about you, so be careful who you give your card to.
It’s an offence to lend a WA Photo Card or allow a WA Photo Card to be used by another person.
Only an authorised person under the Liquor Control Act 1988 can confiscate a card. The authorised person is only permitted to confiscate a card from a juvenile (a person under the age of 18) if they suspect the card is forged, false or counterfeit.
WA Photo Cards should only be scanned or copied with your permission.

WA Photo Card concessions
You may be eligible to receive a concession of up to 100% on the fees payable for your WA Photo Card.

How to apply for an additional WA Photo Card
Cardholders have the option of applying for 2 WA Photo Cards – 1 showing their address and 1 without it.
If you want to order an additional card, with or without your address showing, you need to complete a Photo Card Application form (PC1) and select which card you are applying for.
You need to follow the same steps as indicated in the How to apply application process.
A fee will be applicable.
How to apply for a replacement WA Photo Card
If you need to apply for a replacement WA Photo Card you must attend a Driver and Vehicle Services centre, DoT regional office or agent in person.
You’ll need to provide the following proof of identity documents:
- 1 primary, and
- 1 secondary.

Note:
- You can use a previous WA Photo Card photo and signature (valid for 10 years).
- These images cannot be used for a WA driver’s licence document or learner’s permit card. Similarly, a WA driver’s licence or learner’s permit image can’t be used for WA Photo Card
- Holders of an existing WA proof of age card will need to provide proof of identity when applying for a replacement WA Photo Card.
Frequently asked questions
WA residents aged 16 years and older can apply for a WA Photo Card.
A WA Photo Card is valid for five years.
If you are in possession of an aged pension, a 100% concession will apply. Your concession card must be presented at the time of application. For more information regarding Seniors, Veterans and Pensioner Concession entitlements, please visit Concessions.
The WA Photo Card will have the same integrity as a driver’s licence so it should be accepted by organisations that accept a WA driver’s licence as proof of identity.
You must notify DoT’s DVS within 21 days, to obtain a replacement card, you must complete a PC1 and lodge it in person with primary and secondary identification. A replacement fee will be applicable.
You must notify DoT within 21 days, if you can change your address.
Change of name can only be affected by attending a DVS centre or regional DOT centre or agent in person with documents that support your change of name. Refer to Change my name for more information.
